Updating, closing and transferring files
Users are responsible for informing Customer service of any update to their records.
Users must inform Transit services of:
- Their new address;
- Their new phone number;
- Their new email address (if applicable);
- Any changes to their medical condition;
- Any changes to their mobility aids (manual or electric wheelchair, three-wheeled scooter, four-wheeled scooter, etc.);
- Any new accompaniment needs;
- Any changes in the people who are legally responsible for them (if applicable);
- Any other relevant information.
The eligibility committee must review any change to the level of eligibility or type of accompaniment. A professional must evaluate all details of the diagnosis, deficiencies, disabilities and transit needs."
A user’s file can be closed for the following reasons:
- If the user has died, someone may ask for their file to be closed.
- A user may ask to close their file if they are no longer using the service.
If a user does not use paratransit for six consecutive months, their file will be automatically closed.
A user who would like to use the service again after their file has been closed must fill out a new eligibility application.
If the user moves out of MRC Brome-Missisquoi, they must send a written request to MRC Brome-Missisquoi's Transit services to transfer their file, and Transit services will send the user’s file to the appropriate paratransit service.
