Eligibility application
You must submit an application to Transit services to sign up for paratransit.
Only the original copy of the form signed by the applicant is considered valid by the eligibility committee.
- Print and fill out part 1 of the Application for Paratransit Eligibility form (see the Useful links and documents section below).
- Have part 2 filled out by a professional in the health care or school system who has access to the applicant’s diagnosis.
- Mail the completed form to:

If there are no problems with the application (i.e., the form has been duly completed and signed), the applicant will receive a written decision within 45 days of receipt of their application.
Applicants eligible for paratransit will be mailed:
- A letter of attestation
- The User Guide (see the Useful links and documents section below)
- The fare table
Incomplete or unreadable applications will be returned to the applicant, resulting in a longer application processing time and delayed access to paratransit. The applicant must ensure that they have all the necessary documents and signatures before they resubmit their application.
